(1.) How to Play. Register to win by completing an entry form found on pHisoderm products, while supplies last, or call toll free: 1-800-366-9022 to enter by phone. You can enter online at www.mtv.com. You can also enter by typing or hand-printing the following information on a 3" x 5" card: name, address, phone number, age, and e-mail address (if available). Mail your entry to: pHisoderm/MTV "Real World" Sweepstakes, 1878 East Avenue, Rochester, NY 14610. Only one entry per household. Sweepstakes begins on 3/15/02, and ends on 7/10/02. All entries must be received by 7/15/02. Prizewinners will be selected in a random drawing from all eligible entries received, on or about 7/18/02. MTVN assumes no responsibility for incorrect or inaccurate On-Line Entry information whether caused by a participant or by any equipment or programming associated or utilized in this Sweepstakes or by any human error, which may occur in the processing of the On-line Entries. MTV Networks, is not responsible for any problems or technical malfunction of any telephone network or lines, computer on-line systems, networks, servers or providers, computer equipment, software, failure of email or media on account of technical problems or traffic congestion on the internet or at any web site or combination thereof, including injury or damage to participants or to any other person's computer related to or resulting from participating or downloading materials in this Sweepstakes. If, for any reason, the Sweepstakes is not capable of running as planned, including modification or cancellation in part or in its entirety, infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, or any other causes beyond the control of MTVN which corrupt of affect the administration, security, fairness, integrity or proper conduct of this Sweepstakes, MTVN reserves the right in their sole and unequivocal discretion to cancel, terminate, modify or suspend the Sweepstakes due to any reason of a technical, creative or other nature in their sole and unequivocal discretion and then award the prizes by random drawing among all Eligible Entries received prior to cancellation, termination modification or suspension of the Sweepstakes.
(2.) Prizes and Odds of Winning. There is one (1) Grand Prize. Winner gets to invite 5 of her closest friends to spend the weekend at the MTV "Real World" House in Las Vegas. Grand Prize trip includes round-trip, coach airfare for 6 from a major airport closest to winner's home to Las Vegas, a 2-night stay in the "Real World" House, a catered dinner one evening, and cash spending money of $250 for each of the 6 travelers for shopping, show tickets, or additional, incidental expenses. Approximate Retail Value of Grand Prize trip is $6,000. One hundred (100) Second Prize winners will receive a cool MTV backpack filled with pHisodermŪ products, each having an ARV of $30. Total ARV of all prizes to be awarded is $9,000. Odds of winning will be determined by the total number of eligible entries received.
(3.) How to Claim Prizes. The Grand Prize winner will be announced during the intermission of the MTV Original Movie: "Real World: The Lost Season", on 8/6/02, and will be subsequently confirmed by phone, and in writing. All Second Prize winners will be notified by mail on or before 8/30/02.
(4.) General Conditions. Offer open to all female US residents, between the ages of 13 - 27 years of age. Employees (and the immediate family members of, or persons living in same household) of Chattem, Inc., its affiliates, contractors or vendors, are not eligible to participate. Void where prohibited/restricted. All federal, state and local laws & regulations apply. Grand Prize winner must execute an Affidavit of Eligibility and Liability/Publicity Release, (TN residents need not sign publicity release) to be returned within 3 days, or prize will be forfeited, and an alternate winner selected. Travel companions of winner will also be required to sign a travel/liability waiver before airline tickets will be issued. Parent or guardian of any minor traveling must sign a consent form. Prizes may not be substituted, transferred or exchanged for cash, except at the sole discretion of the Sponsor. Taxes on all prizes are the responsibility of winners. Sponsor and MTVN are not responsible for illegible, damaged, lost, late or misdirected mailed entries, or game materials. All submissions become property of Sponsor. All winners agree to release Sponsor, MTVN and its respective affiliates, subsidiaries, employees, officers, directors, suppliers, distributors, and vendors from any and all liability for injuries, losses or damages sustained in connection with participation in this game or the possession, acceptance or use of any prize awarded. By accepting prize, winners also consent to the use of their names, pictures and/or other likeness for publicity and promotional purposes (except where prohibited) as determined by Sponsor, without additional compensation. By submitting an entry, you accept and agree to be bound by these Official Rules and all decisions of The National Sweepstakes Co., an independent judging organization. All prize awards are subject to verification of eligibility.
